10 mg vial + 2 mL BAC = 5000 mcg/mL; 250 mcg = 5 U, 500 mcg = 10 U on U-100. NOTE mg vs mcg = 1000x hazard (human studies dosed in mg).
Titration
Start ~200-250 mcg/day, increase toward 500 mcg if tolerated (no clinical basis)
